Note to referring physicians: A detailed referral question, your assessment of urgency, relevant history and any relevant reports or documents are very appreciated and helpful in triaging your patient. Referrals are screened and triaged on receipt and we will advise you within one week of the approximate wait time for an appointment OR if there will be any delay in triaging (due to holidays or office closure). We will also promptly advise you by fax & telephone if the referral cannot be accepted for any reason.
Note to Patients & Families:
We will contact you directly to set up a first consultation appointment. In order to use our time together most effectively, we may ask you to complete a patient history or other form in advance and fax this to us or bring it with you, along with any relevant reports and past assessments.
The first appointment normally lasts 75-90 minutes. Please plan accordingly.
We provide an electronic reminder by phone, text or email (your choice) 2-3 days before the appointment. If unable to attend your appointment, please advise us as soon as you know so that we can offer the time to another patient on our waitlist. It is our policy not to rebook patients who miss an appointment without appropriate advance notice.
